Definitions for Common Terms in Quick Connect.

How to Create a Request:
Intent of this process is for the designated agency supervisor initiate a request to process one of the following types of application within a specific existing case ID in Quick Connect. The following types of application:

  1. renewal
  2. amendment
  3. withdrawal 

Family:

What is the Family required to do:

  • Renewal: Gives the applicant I family the ability to update their application. Upon completion the family can submit the application by pressing 'submit' on their review screen.
  • Amendment: Gives the applicant the ability to update their demographics based on the type of amendment being processed (change of address)
  • Withdrawal: Gives the family the ability to withdraw or close their application or current foster I adult license.

***Family is ONLY able to process their application if a user name has been issued to them by the designated agency system administrator.  If not, it is the agency workers responsibility to input the information based on a hard copy application that the family/applicant filled out.

Agency or OLCR:  

  1. Gives the assigned agency licensing worker the ability to complete the home study while the family completes the application.
  2. Agency licensing worker have the ability to request the CPS / APS background check and review its results prior to submitting the application to OLCR
  3. Agency licensing worker have the ability to request the Life Safety Inspection and review its results. If the outcome of the inspections determines any deficiencies, the agency worker can input follow up dates and its results in assuring that the home is now in compliance per foster care rule.
  4. View and print out the approved license once it has been approved by the Licensing Authority, OLCR
  5. Ability to print out required documents to be signed by the family.

Agency Licensing Supervisor is the only role that has permission rights to create a Request.  Agency Systems Administrator and Agency Licensing Specialist have no access or permission rights to initiate this process.

  • Step 1: Log on to the Family Home Licensing web site (Quick Connect) 
  • Step 2: Select the last approved application
  • Step 3: Amendment Type
  • Step 4: Withdrawal Request

Step 1:

Log on to the the Family Home Licensing web site (Quick Connect)

Step 2:

  1. Search the case name on the search screen window. You can search by inputting only or a combination of the following parameters.
    1. Licensee ID #
    2. Last Name and First Name of primary licensee  
  2. Uncheck / Unclick the Open only box. This allows the application to search the last approved status or application.
  3. Click on 'Search'
  4. Once you have found the correct case name or ID, select the most recent or last approved license that has been issued or approved by OLCR. You can decipher this by the following approved substantive review statuses:  
    1. Substantive Review Completed / Approval Letter
    2. Substantive Review Completed / Approval with Modification Letter
    3. Substantive Review Completed / Approval Letter with Provisional License

  5. Select the last approved application by 'clicking' on it. By selecting correctly the most recent approved application, this gives Quick Connect the ability to identify what case or family you are working on to initiate your renewal / amendment or withdrawal.  
  6. Upon selection Quick Connect will return to the 'search' screen.  
  7. Go to the menu header of 'Amend/Withdrawal' and select Create a Request.

  8. Once you have selected the 'Create Request' drop down menu it will forward you to the main 'Create Request' Screen.

    A 'select' drop down box under Request Type exist. This provides you the option to select on the types of application that you want to process:
    1. initial/renewal
    2. amendment
    3. withdrawal

      ***Initial/Renewal: This alleviate the subjectivity of what type of an application needs to be processed when a  licensed family has been closed for a period of time.  Quick connect will designate the following application type.  If the home has been close;
      • Less than 6 month and 1 day: Renewal
      • 6 Months and 1 day or more: Initial
  9. Once you have selected the 'type of application' you are working on. Press Save.
  10. Quick Connect will provide a prompt stating 'requested created' or 'unable to create request.'
    If 'unable to create request' appears:
    Recommendation I Solutions:
    • Please check if a current 'open request exist' that has not been submitted in the main search screen. Please utilize the 'open only' feature upon searching entering your search criteria/parameters.

Step 3:

Amendment Type:

If you are requesting to process an amendment the following types of amendment will appear. Please select the type(s) of amendment you are processing. Your selection is not limited to one (1) type. After selection, Press Save.

Step 4:

Withdrawal Request:

If you are processing a closure of an existing license or an initial application, please select the 'withdrawal" type.  Upon selection this type of application the following withdrawal request screen will appear.

Types of withdrawal action:

  • I voluntarily withdraw my application for a license to operate a foster home or an adult developmental home.
    Action Type: Applies to an initial applicant who wishes to not pursue licensure on an application that was not submitted to OLCR.
  • I voluntarily close my application to operate a foster home or an adult developmental home.
    Action Type: Applies to an existing licensed foster care provider who wishes to close their existing foster/adult license.
  • Applicant wishes to withdraw an application for a license to operate a foster home or an adult developmental home.
    Action Type: Utilize this action when an agency closes an initial application on behalf of the applicant or family.  Agency needs to have one of the following supporting documentation in the file:
    1. Formalized letter for the agency noting closure of application on their behalf
      OR
    2. Signed withdrawal form with applicants reason for not pursuing licensure
  • Licensee wishes to close a license to operate a foster home or adult developmental home.
    Action Type: Utilized when an agency closes an existing foster/adult license on the family's behalf.  Agency needs to have one of the following supporting documentation in the the file:
    1. Formalized letter for the agency noting closure of application on their behalf
      OR
       
    2. Signed withdrawal form with applicants reason for not pursuing licensure
    3. Licensee failed to renew license.
      Action Type: Utilized by either OLCR or agency when a licensee/family fails to complete their application in a timely manner.  If OLCR processes closure, a formalized letter will be sent to the family and a copy to the agency.
